Identify triggers: You and your therapist or Case Manager should work together to identify situations, events, or emotions that may trigger the client’s crisis.
Warning signs: You should identify physical, emotional, or behavioral warning signs that indicate you may be in crisis.
Coping strategies: You should identify coping strategies that have worked for you in the past, such as mindfulness, deep breathing exercises, or physical exercise.
Support network: You should identify individuals you can reach out to for support, such as friends, family members, a therapist, or your Case Manager.

Safe environment: You should identify a safe environment you can go to if you feel unsafe, such as a friend’s house or a public place.
Resources: Your should identify resources that can provide you additional support, such as support groups offered at EYES INC of NJ, or online resources.
Review and update: Your safety plan should be reviewed regularly with your Counselor or Case Manager to ensure it is up to date and effective in helping you stay safe.
